Headset Play Is the Knock You Feel Through the Bars
A small knock when you brake is the front end telling you something has come loose. Ignored, it wears the bearing races until they are ruined.

Both approaches to headset adjustment work. What differs is what they cost you, and the cost is what this sets out.

What the headset actually does
The headset is the pair of bearings that lets the fork rotate inside the frame's head tube while carrying the loads from the front wheel. It has to allow smooth rotation and simultaneously resist being pulled apart or pushed together by braking and impact forces.
That is achieved by preloading the bearings, which means clamping the assembly with just enough force to remove play without binding. Too little preload leaves the fork free to move a fraction of a millimetre, which you feel as a knock. Too much preload loads the bearing surfaces continuously, which makes the steering heavy and shortens the bearing's life.
Finding the play
The standard check is to hold the front brake on, stand beside the bike and rock it forwards and backwards while feeling the joint. A hand resting where the fork meets the frame will feel movement that is too small to see. A knock felt when braking hard, or when riding off a kerb, is the same fault appearing under load.
The bit that actually matters: brake calipers with play in their mounting can produce a similar knock, so lifting the front wheel and turning the bars helps separate the two. If the bars turn with a notchy feel or catch in the straight-ahead position, that is bearing wear rather than adjustment.
How preload is set on a threadless system
On a threadless steerer, the top cap bolt pulls the stem down against the spacers and the bearings, setting the preload. It does not clamp the stem to the steerer, and it is not a structural fastener, which is why it takes very little force. The stem's own side bolts are what clamp everything in place once the preload is correct.
With a loaded rack, the order matters, since tightening the stem bolts first means the top cap cannot move anything and the adjustment does nothing. There must also be a small gap above the steerer tube for the system to have anything to pull against, which is a detail that catches people out.
Threaded headsets and locknuts
Older and many utility bicycles use a threaded steerer with an adjustable race and a locknut above it. Preload is set by the adjustable race and then held by tightening the locknut down against it.
The classic mistake is adjusting the race without loosening the locknut first, which simply moves both together. It is also possible to have the adjustment correct and the locknut loose, which lets everything drift out of adjustment while you ride.
Two spanners are usually needed, and the system is entirely serviceable at home with basic tools.
When it is worn rather than loose
A headset that has been ridden loose develops indentations in the bearing races where the balls have been hammered under load. The symptom is a steering that wants to centre itself, catching slightly as it passes straight ahead. At that point adjustment cannot fix it, because the damage is in the surfaces and tightening only makes the notching more obvious.
Replacing pressed-in bearing cups needs specific tools and is the point at which most riders hand it to a workshop. Cartridge bearing headsets are simpler to replace, since the whole bearing lifts out and a new one drops in.
Why this one is worth catching early
The steerer tube, the fork and the headset together form the assembly that holds your front wheel and takes your braking loads. A knock is cheap to fix, a notched race is a moderate repair, and a cracked steerer is a catastrophic failure at the worst moment. Any crack, visible damage or creaking from the fork or steerer is a stop-riding situation for a qualified mechanic to assess.
Come the wet months, carbon steerers and lightweight forks have specific clamping and torque requirements that are not guesswork. Checking for play takes ten seconds and is worth doing whenever you have the bike upside down for anything else.
