A pad worn thinner at one end than the other is being presented to the braking surface at an angle rather than flat. On a rim brake, that usually means the pad holder needs realigning so the whole face contacts the rim squarely.

On a disc brake, tapered wear along the pad often points to a caliper that is not centred over the rotor. With a loaded rack, contamination by oil, chain lubricant or degreaser produces a similar loss of bite and is far harder to reverse. Once a pad has absorbed oil, cleaning the surface rarely fixes it for long, because more oil migrates out as it heats.
Brakes are safety-critical, so a brake that has lost bite and does not recover is a reason to see a mechanic rather than to experiment.
Knowing when they are finished
Most pads carry a wear indicator, either a groove that disappears or a minimum thickness marked on the backing. Running a pad down to the metal backing means metal against your rotor or rim, which turns a cheap job into an expensive one.
